The calendar asks for your address, because the appointment happens there. I need floor space roughly the size of a parked car and a power outlet within reach. Everything else comes with me: table, sheets, bolsters, music.
A confirmation from the calendar, and a separate note from me with the intake form. Filling it in beforehand turns the first half hour from paperwork into assessment. That is the half hour you are paying for.
